Cusick was of American Indian ancestry. He apparently had a problem with alcohol, as he enlisted in 1908 but didn't earn a Good Conduct Medal until 1923. He is buried at Beverly National Cemetery in New Jersey.
The Good Conduct Medal and Mexican Service are re-ribboned but have original brooches. Connecting link on hanger bar of GCM has been replaced by twine.
Location of 1915 Haitian Campaign Medal is unknown.
